What is the Intake Process?
Step 1: Contact Ms.Blattner by email or phone. She can provide up to a one-half hour free phone consultation to explain our services in more detail. It works best to schedule a specific time for this.
Step 2: After speaking with Ms. Blattner, complete the Intake Packet and return to
Ms. Blattner electronically or by mail. At that time, an appointment will be
scheduled for an intake meeting at our office in Kensington.
Step 3: The goal of the intake meeting is to review the documentation and to
create a road map for moving forward. Your referral concerns as outlined
on the intake forms will provide guidance on decision-making
for the best way to have your needs met.
Step 4: At the time of the intake meeting, your
payment options will be discussed. Please refer
to the packet for more details about fees.
